HIPAA Notice of Privacy Practices

This notice describes

How protected health information about you may be used and disclosed by the clinicians and pharmacies that provide your care through Zappy, and how you can get access to this information. Please review it carefully.

Our responsibilities

We are required by law to: maintain the privacy of your protected health information; provide you this notice of our legal duties and privacy practices; abide by the terms of the notice currently in effect; and notify you in the event of a breach of unsecured protected health information.

Uses and disclosures of your health information

Treatment

We use and disclose your health information to provide treatment — for example, your clinician reviewing your case, the pharmacy filling your prescription, and lab providers running ordered tests.

Payment

We use and disclose your information to process payment for services. We do not bill insurance.

Operations

We use and disclose information to operate the practice — for example, quality reviews, training, and licensing oversight.

As required by law

We may disclose your information when required by law — for example, court orders, public health reporting, and FDA adverse event reporting.

Your rights

  • Inspect and copy. You have the right to inspect and request a copy of your medical records.
  • Amend. You may request a correction to your records.
  • Accounting of disclosures. You may request a list of disclosures of your information.
  • Restrictions. You may request restrictions on certain uses or disclosures.
  • Confidential communications. You may request that we communicate with you in a specific way (for example, by mail rather than email).
  • Paper copy. You may request a paper copy of this notice at any time.

How to exercise your rights

Submit requests in writing to privacy@zappyhealth.com. We respond within 30 days.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at privacy@zappyhealth.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

We reserve the right to change this notice. The notice currently in effect is posted on this page with a "last updated" date.
