Minoxidil + Spironolactone

Topical minoxidil + spironolactone for women's pattern hair loss — the prescription-strength combo most clinics gate behind dermatology visits.

Prescriptions require an online visit reviewed by a U.S.-licensed provider. Compounded medications are prepared by licensed U.S. pharmacies and are not FDA-approved. Individual results vary.

Who it’s for

  • Mild-to-moderate acne, melasma, or texture concerns
  • Want prescription-strength formulas matched to skin type
  • Willing to commit to daily SPF and 8–12 week consistency

Most women notice it in the part first — that line where the scalp shows — and only start saying it out loud months later.

how it works

Topical (or low-dose oral) vasodilator that prolongs the anagen growth phase of hair follicles. Foundational therapy for both male and female pattern hair loss.

What we can and can't tell you

The individual ingredients are well established; head-to-head trials of this exact compounded combo don't exist. And minoxidil is maintenance therapy — the gains hold while you use it, not after you stop.

  • Who makes it A LegitScript-verified 503A compounding pharmacy, from your prescription — never a gray-market or overseas supplier
  • To what standard USP <797> sterile compounding and USP <800> hazardous-drug handling, where applicable

what the first months look like

1
First weeks

Quiet phase. Follicles are shifting from resting (telogen) into growth (anagen) — nothing visible yet, and that's normal.

2
Weeks 4–8

Initial shedding. Resting hairs release to make room for new growth — it looks like the opposite of progress and is actually the sign it's working. Don't stop here.

3
Early months

Regrowth phase. Hair grows slowly by nature — judge progress in monthly photos, not the daily mirror.

4
Ongoing

Maintenance. Consistency is the whole game: the follicles stay in growth phase as long as the signal is there.

Dosing, step by step

Minoxidil (topical) 1 mL of solution to the scalp twice daily — 2% or 5% for women — your clinician sets the strength
Spironolactone (topical) Typically 5% formulation, once daily — applied to the affected scalp
Oral minoxidil (alternative) 0.25–2.5 mg daily — clinician-supervised option if twice-daily topical isn't sticking

If topical adherence is the problem, low-dose oral minoxidil is a real alternative — but only under clinician supervision.

And when it gets rough

  • Scalp irritation Usually mild and settles with continued use. If it's persistent or spreading, message your clinician — a strength or formulation change often fixes it.
  • Initial shedding (weeks 4–8) Expected and temporary — it's old resting hairs clearing out. Stopping now restarts the clock.
  • Dizziness or lightheadedness Uncommon with topicals, but report it — especially if you take blood-pressure medication, since minoxidil can add to its effect.
  • Menstrual changes or breast tenderness Known with oral spironolactone and unexpected on topical — flag it to your clinician promptly.

Why we start you here
We start most women here because it works on two fronts at once — one eases the hormonal pull on the follicle, the other coaxes resting follicles back into a growth phase — and you shouldn't need a months-out dermatology referral to get it. The thing we flag hardest, before anyone starts: hair often sheds a little more before it grows, usually around the second month, and that's exactly when people want to quit. We'd rather you hear it from us first, so the shedding reads as the follicle cycling, not the treatment failing. These are real prescriptions, not cosmetics, so we check in early and adjust — especially if we're using the oral route.

Questions, answered.

How fast will my skin change?
Texture improves in 4–6 weeks; pigment and acne typically resolve over 8–12 weeks of consistent use.
Can I use this with retinol?
Your clinician compounds a single formula that replaces over-the-counter retinol — using both isn't necessary.
What about sun exposure?
Daily SPF 30+ is non-negotiable on prescription topicals. We include a primer recommendation in your plan.
