The honest version

Identical medicine to the Zepbound pen — the trial data is the same because the drug is the same. The vial costs less because you draw the dose yourself.

what the first months look like

1
Weeks 1–4

Priming dose (2.5 mg).

2
Weeks 5–20

2.5 mg steps ~monthly toward maintenance.

3
Month 5+

Maintenance at 10–15 mg — where the week-72 trial numbers live.

Dosing, step by step

Ladder 2.5 mg start, +2.5 mg ~monthly to 10–15 mg — label-standard

Each weekly dose is drawn from its own single-dose vial with a syringe. Your clinician walks you through the first draw on a video visit.

And when it gets rough

  • Nausea Mechanical, dose-tracked, fades with adaptation — pace the steps with your clinician.
  • Injection-site reactions Rotate sites; mild and local is normal, spreading redness is a call.
  • Unsure about drawing the dose Normal in week one. We'll do it with you on video, and switching to the pen is a message away.

Safety information

Every prescription is reviewed by a U.S.-licensed clinician who decides whether this medication is appropriate for you. Tell them during your visit if any of the following applies to you or your family.

In rodent studies, semaglutide and tirzepatide caused thyroid C-cell tumors. It is not known whether they cause these tumors, including medullary thyroid carcinoma (MTC), in people. Do not use this medication if you or any of your family have had MTC, or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

Why we start you here
The vial and the pen are the same tirzepatide from the same manufacturer — the difference is that you draw the dose yourself with a syringe instead of clicking a pen. For some people that's a dealbreaker and we put them on the pen without argument. For plenty of others it's a two-minute step they stop thinking about by week three, and the money saved is the reason they can stay on treatment long enough for it to matter. We'll walk you through drawing your first dose on a video visit, and if it turns out not to be for you, switching to the pen is a message, not a new prescription.

I love Zappy Health!! They started me on my GLP-1 journey in February of 2024! They helped me when my own doctors wouldn't! I have exceeded my goal weight and have a healthy BMI of 22! I started at 49! Thank you Zappy for all your kindness, and your support. I will be forever grateful.

I do my online survey every month to let them know about my weight loss and I've received my injections every month. Flat rate, no hidden fees, and the cost doesn't increase. This company is great and helps you every step of the way.

I had read many reviews and decided to give Zappy my business. I lost the needed weight and feel so much the better for it. I received all of my medication on time. I do feel this company has a high regard for each client they treat. They deserve the 5-star rating and I highly recommend them.

Questions, answered.

Do I need insurance to start?
No. Zappy is cash-pay; FSA/HSA cards are accepted for eligible expenses.
How fast will I see results?
Most members notice reduced appetite within 1–2 weeks; weight changes typically start at week 4–8 with steady titration.
What if I have side effects?
Your clinician adjusts the titration. Nausea, fatigue, and constipation usually resolve as the body adapts.
